Urgent Care

Help with urgent needs such as:

  • Injuries such as sprains, cuts, or bruises.

  • Illnesses such as the flu, cold, cough, fever, sore throat, and minor headaches.

Diabetes Management

Helping patients manage their diabetes includes monitoring and not only helping maintain blood sugar through medications. TeamCare Medical works with nutritionists, physical and occupational therapists, and therapeutic recreation professionals to help patients with lifestyle changes and making better choices for their health.

TeamCare Medical’s staff is experienced and knowledgeable in the diagnosis and treatment of Chronic Obstructive Pulmonary Disease (COPD) and other pulmonary conditions in older adults.

Blood Pressure

TeamCare Medical’s personalized approach to care means we strive to recognize the risks and signs of diseases, such as high blood pressure, before it gets worse. The risk of hypertension gets higher as a person ages — and our patients rely on us to help them manage their condition with medical treatments and other interventions such as education and referrals to rehabilitation therapists, nutritionists, and others.

Our goal is to help our patients with early diagnosis and treatment of diseases, while ensuring their needs, goals, and preferences are respected and taken into consideration.

TeamCare Medical works with adults 55+ to recognize early signs of heart failure to help our patients seek the help they need timely. This includes education on signs and symptoms, regular check-ups, and coordinating necessary treatment plans and referrals to specialists.
