TeamCare Medical will be opening soon for individuals ages 55+ at 629 Park Place, Brooklyn, NY 11238. Most insurances accepted. We offer healthcare services to adults 55+ to help them continue to age in place. Our goal is to provide individualized whole-person care that focuses on older adults’ physical, emotional, social, and mental wellbeing and help manage their complex needs. We believe that personalized care means understanding the needs of our patients in all aspects of their healthcare journey, and that healthcare goals and decisions may change significantly as we age. Located in the heart of New York, our team is dedicated to getting to know those we serve and what matters to them, understanding their personal values and traditions, and prioritizing person-centered care.
