TeamCare Medical’s expert team has a depth of experience in managing acute and chronic illnesses —with a focus on medical monitoring and providing care that takes into consideration not only our patients’ physical health, but also their social, emotional, and mental wellbeing, as well as the challenges and considerations unique to the individuals we serve. Our expert team also conducts lab testing on site.

Preventative Care

Our goal is to help our patients with early diagnosis and treatment of diseases, while ensuring their needs, goals, and preferences are respected and taken into consideration. We encourage individuals to get yearly check-ups and keep up to date with screenings such as mammograms, prostate exams, colonoscopy, and others to help prevent any condition from getting worse, as well as unnecessary, avoidable hospitalizations, rehospitalizations, and emergency department visits.

TeamCare Medical offers vaccinations for Influenza, COVID-19, Pneumococcal, Pneumonia, Shingles, tetanus, RSV, and others.
