TeamCare Medical works with organizations such as CenterLight Healthcare’s Program of All-Inclusive Care for the Elderly (PACE), one of the oldest and the largest single-state PACE in the nation and an organization focused on providing everything their participants need to remain living safely and independently in their own homes. Through this collaboration, our team is able to help older adults receive all the care they need by being a part of their dedicated care team.
